In an effort to provide consumers with adequate information necessary to make an informed choice about placement, ANH sent every facility a detailed facility questionnaire to complete. The information requested is divided into seven (7) sections: contact information, types of residents accepted for admission, bed types, physical plant, services offered, staffing information, and business information.

 



This page includes the contact information for the facility, including, where applicable, the person who completed the facility questionnaire.

If the facility did not respond to the ANH questionnaire, a statement indicating this is included on the page.
